The evaluation of the efficacy and tolerability of ,,atypical" neuroleptics in clinical and out-patient use cannot be extrapolated from results of controlled clinical studies. An alternate approach for recording and analysing adverse drug effects (ADE) was initiated by the Arbeitsgemeinschaft Arzneimitteltherapie bei psychiatrischen Erkrankungen (AGATE) (i.e., Working Group on Drug Therapy in Psychiatric Disorders). In the period from 1993 to August 2005, ADE of 2,002 patients were recorded and analysed in the AGATE database. The analysis was performed for olanzapine, risperidone, amisulpride, quetiapine, clozapine and haloperidole as reference substance, both for single or combination therapy. Numerous different ADE were recorded, among others also extrapyramidal effects and disturbances of liver function. Hereby, no dose-effect correlation could be derived. In conclusion, a substantiated evaluation of a potential relation between ADE and an individual neuroleptic drug requires a systematic registration of ADE and a determination of drug plasma levels.
