Abstract

Objective: The study aimed at evaluating the psychopharmacological pre-treatment of schizophrenic inpatients referred by psychiatrists in private practice (PP) as well as general practitioners (GP) in order to show the use of atypical antipsychotics as well as clues for quality improvement. Methods: Data of the German psychiatric basic documentation system (DGPPN-BADO) of all schizophrenic inpatients (n = 578) admitted to a psychiatric hospital in 2003 were analysed. Results: 10.7% of patients were referred by a PP, 14.2 % by a GR 90.6 % of PP-patients (GP: 84.1 %) had a psycho-pharmalogical pre-treatment, antipsychotics were prescribed to 77.7 % of PP-patients (GP: 81.2 %). Conventional antipsychotics were given to 50.9 % of PP-patients (GP: 49.3 %) atypicals to 39.6 % of patients (GP: 44.9 %). Antidepressants were prescribed to 22.6 % of PP-patients (GP: 18.8 %), with a similar ratio of tricyclic antidepressants and selective serotonin reuptake inhibitors. 17.0 % of PP-patients (GP: 18.8 %)received mood stabilizers, 9.4 % of PP-patients (GP: 4.3 %) tranquilizers. Conclusions: In general, the psychopharmacological treatment of schizophrenic patients by PP and GP differed only slightly. However, the antipsychotic treatment could further be optimized with respect to current treatment guidelines.
