Psychopharmacotherapy of depressive patients before their admission to a psychiatric hospital

Abstract

Objective: The aim of the study was to investigate the outpatient psychopharmacological treatment of patients with depressive disorders referred by psychiatrists in private practice (PP) and general practitioners (GP). Methods: Data of the German psychiatric basic documentation system (BADO) of all depressive inpatients (n = 1970) admitted consecutively to the Department of Psychiatry from 2002 to 2003 were analysed. Results: 22.4% of the inpatients were referred by a GP, 18.4% by a PP. 61.9% of GP-patients (PP: 82.5%) had a chronic and recidivic, respectively, course of illness. GP-patients had significantly more seldom a psychopharmalogical treatment before admission (59.4% vs. PP: 89.7%; p < 0.001): GP prescribed significantly more seldom antidepressive drugs (53.1% vs. PP: 87.9%; p < 0.001) and antipsychotics (9.4% vs. PP: 25.9%; p < 0.016). Tranquilizers (GP: 15.6% vs. PP: 13.8%) and mood stabilizers (GP: 7.8% vs. PP: 13.8%) were in use in similar amounts. The use of psychopharmaceuticals by GP from 2000 until 2003 fluctuated, whereas the prescription of antidepressive drugs by PP increased by 24% (p=0.015). Conclusion: The study points to GPs' deficits concerning the psychopharmacological treatment of depressive disorders. Early recognition and adequate treatment of depressive patients in primary care should be a main objective of psychiatric education for GP.
