Anaphylactic reaction to fizzy multivitamine tablets in a patient with gelatine allergy

Abstract

We present a 41-year-old woman who developed urticaria, facial edema and dyspnea 30 minutes after drinking a fizzy multivitamine drink. Prick-to-prick-test with the fizzy multivitamine tablet was positive. In the following, prick-to-prick-tests were carried out with each ingredients of these fizzy multivitamine tablets. We found a positive reaction for gelatine (porcine) which has not been declared on the fizzy multivitamine tablets. Anaphylactic reactions towards gelatine have been described recently for gelatine-containing plasma substitutes and vaccines, especially measles-mumps-rubella-vaccines. Primarily histamine release was suggested to be the main cause of anaphylactoid reactions towards gelatine-containing plasma substitutes but in the following IgE-mediated reactions have been reported for different gelatine-containing vaccines and plasma substitutes. Anaphylactic reactions towards gelatine-containing food seem to be rare. it might be suggested that digestion affects the allergenicity of gelatine. Even if anaphylactic reactions towards gelatine-containing food seem to be rare, in case of unclear allergic reactions towards food or oral medications an IgE-mediate reaction to gelatine should be taken into consideration.
