Beside frontotemporal dementia (FTD) and progressive nonfluent aphasia (PA), semantic dementia (SD) is supposed to form part of the spectrum of frontotemporal lobar degeneration. The typical feature of SD is slowly progressive language disorder initially and throughout the disease course that remains relatively isolated from other cognitive or behavioural deficits. Brain imaging reveals frontotemporal lobar atrophy or reduction of blood flow respectively. The present case history of a 84-year-old women exemplifies the insidious onset with early decline of loss of word meaning and fluent speech with semantic paraphasias. Activities of daily living were preserved until advanced stage of the disease, and at lost cognitive impairment emerged. In addition to bilateral cortical volume reduction on CCT, SPECT shows unilateral perfusion defects left frontotemporal. This case study and the literature reviewed suggest that SD should be considered as a relevant differential diagnosis of both aphasias and dementias.
