Abstract

Patients with opioid and concomitant medication are seen routinely in the peri-operative setting. Some specific problems relating to these patients need to be addressed, e. g. drug interactions, side effects and the correct dosing of anaesthetics. Chronic pain patients will present different problems than patients who are in an opioid maintenance pro-gramme. If possible, the patients' medication should be continued. Regional analgesia should be incorporated into the anaesthetic regime whenever possible. If regional analgesia is not an option, an increased need of opioid medication should be expected, also a large interindividual variability. The opioid doses needed to provide sufficient postoperative analgesia might vary significantly. Because of the additional postoperative pain, the dose will need to be adjusted. Withdrawal symptoms caused by the cessation of the patients' medication must be avoided.
