Abstract

Within the last 20 years, PTCA has become one of the most important therapies for coronary artery disease, immediate success may be achieved in over 90% of cases. The most important acute complication, especially in high risk patients and in complex lesions, is abrupt vessel closure. Nowadays this can usually be avoided or treated by stent implantation. Also, treatment with new antithrombotic agents (glycoprotein llb/llla receptor antagonists) help to reduce acute complications. Restenosis occurs in 30-50% of cases and remains an unsolved problem. Newer studies suggest that stent implantation may also reduce restenosis rate. Indications for PTCA depend on clinical and angiographic parameters and have to be weighed against medical and surgical treatment with regard to risk, acute success as well as longterm prognosis.
