Since the detection of Huggins and Hodges fifty years ago, that a prostatic carcinoma can be treated by deprivation of androgens of, no progress was really reached concerning prolongation of life time except of some treatmentmodalities. All systemic therapeutic strategies are only palliative and not curative. The symptoms of metastatic P. C. are mostly atypic and similar to usual symptoms of a BPH. The basic principal of the endocrinological treatment of a metastatic P. C. ist based on the deprivation of the androgenstimulus to the cells of the P. C. This can be performed by medical or surgical castration. Only a small group of patients with minimal disease has some profit concerning life time prolongation by complete androgendeprivation. Bone pains caused by metastases of the P. C. can be treated very effectively by radiatiotherapy with the radionuclide Rhenium. The chemotherapy shows overall bad results in the treatment of hormon resistant P. C. The hormonal therapy by androgen deprivation is till now the treatment of choice. As soon as the P. C. is not any more hormon sensitive treatment strategies which improve the quality of life of these patients should be placed in the foreground.
