Recently published controlled multicenter trials indicate, that the conventional step by step therapy in heart failure is no longer the therapy of choice. Drug therapy in heart failure is no longer restricted to symptomatic patients, but is extended to asymptomatic patients with significant left ventricular dysfunction after myocardial infarction (EF < 40%). ACE-inhibitors are the first-line therapy in patients with asymptomatic or symptomatic left ventricular dysfunction. If patients show signs of congestion, the addition of a diuretic would be the next therapeutic step. If heart failure cannot be controlled by a combined therapy of diuretics and ACE-inhibitors, digitalis should be added.
