Infection with the human immunodeficiency virus (HIV) is continuing to increase rapidly throughout the world. In some countries, the final stage of this infection, acquired immunodeficiency syndrome (AIDS), is already the main cause of early death in both men and women. Anonymous, unlinked HIV testing (AUT) recently performed at five Bavarian hospitals revealed 0.3% of all blood samples tested to be HIV-positive. Most of these patients, however, showed no HIV-related symptoms. It must therefore be concluded that medical personnel have contact to a fairly large number of asymptomatic patients infected with HIV The fact that the literature contains reports of about 25 cases of nosocomial HIV infection shows that concern expressed by physicians and nursing staff about their occupational risk of infection by patients is certainly not unfounded. With the aim of providing a better understanding of the problems associated with HIV, this review article provides up-to-date information on the pathogenesis, virology, epidemiology, diagnosis and clinical course of HIV infection. In addition, problems of screening for HIV are addressed and the presently limited possibilities of treating patients infected by HIV are discussed.
