Periodontitis is a risk factor for the longevity of prosthetic restorations. In particular, rehabilitation of severely compromised dentitions with stage IV periodontitis requires sophisticated concepts. This is especially relevant if implants are to be placed. In order to ensure stable conditions in the long term, inflammation-free conditions must be established by successful periodontins therapy. The indivi- dual prognosis for each abutment tooth is essential in treatment planning. Provisional restorations known as long-term temporaries or temporary partial dentures are often required prior to treatment. Subsequent to periodontal pre-treatment, abutment teeth must be selected for supporting the final restoration dependent on their number, position and individual prognosis. Regardless of whether tooth- or implant-supported or hybrid dentures are to be inserted, a large-area support polygon is always favorable. Fixed dentures may have a positive effect on existing degrees of loosening due to the primary interlocking created, but removable dentures can also have a stabilizing effect due to the secondary interlocking they provide. Increasing the number of available abutments with dental implants is also conceivable for periodontally damaged patients, but an increased risk of peri-im- plant infections must be taken into consideration.
