Physicians are required by law to pro vide adequate documentation. Diagnostic findings and measures that are not documented are considered as not existent or not carried out. Those who are responsible for their patients will open themselves up to avoidable litigation risks if they fail to provide adequate do cumentation. This article summarises the most important aspects of the German laws pertaining to medical documentation and includes valuable suggestions for daily practice.
